Health Assessment
A. Monitoring Kindergarten health assessment requirements
B. Conduct vision screening of grades 1, 3, 5, and 7; screen new students; grades 4 and 2 as time allows
C. Conduct hearing screening of grades 1 and 3; also K if not recorded on Health Assessment
D. Assist Dental Hygienist schedule/follow-up for grades K and 5
E. Screen students referred by teachers and/or parents for hearing, vision, scoliosis
F. DEC 3 and yellow folder screening
Health Counseling and Teaching
A. Schedule and conduct 5th grade growing up classes
B. Bloodborne pathogen/communicable disease/diabetes annual update for employees
C. Medication administration training for back-up non licensed personnel
D. Consult with teachers on health matters, train/update diabetes care managers on care plans
E. Provide health education to students as requested by classroom teacher
F. Participate in health promotion activities – hand washing, nutrition, physical activity, safety
G. Participate in 504 and IEP conferences as needed
Record Keeping and Reporting
A. Review of health information upon enrollment
B. Complete state immunization and health assessment report for K students
C. Record screening results, referrals, and follow-up on health cards
D. Write individual nursing care plans when indicated
E. Report suspected child abuse and/or neglect
F. Compile statistics for annual report
G. Complete BBP Annual Update each January
H. Maintain copies of BBP training and Annual Update for 3 years.
